PROGRAM DESCRIPTION: This one day program will feature speakers with various expertise on some healthcare issues. The topics to be discussed are: Care Transition Initiatives, Medication Titration, Continuum Curriculum for HF Cardiology, Diabetes Medications and HF Outcomes and HF Navigation/Outcomes.

LEARNING OBJECTIVES:

Upon completion of this program, the participant will be able to:

Explain the effectiveness of care transitions initiative in assisting quality of care delivery
Recognize the importance of enhancing guideline-directed medical therapy for patients with heart failure with reduced ejection fraction (HFrEF)
Discuss treatment options for specific heart failure classes and stages
Evaluate the literature regarding heart failure outcomes and medications for type 2 diabetes mellitus
Describe the role of the heart failure clinic and nurse navigator in reducing hospital readmissions
